Registration Information
Patient registration is located in the Outpatient Department. Hours of operation are 8:00am to 4:30pm. During these hours, all inpatient, non-emergency admissions are handled through this area.
Patients being admitted to the Hospital through our Emergency Care Center, are registered within the Emergency area.
During the registration process, you will be asked to provide information about your insurance, and to sign a number of consent forms. These forms are important, as they allow our staff to provide the care requested by your physician. Accordingly, they also allow us to bill your insurance company for services rendered.
At times, we realize that registration paperwork may seem redundant or excessive. Please know that we are constantly striving to keep our process streamlined. We appreciate your cooperation.
Beyond the information presented in General Registration, here are some important facts to know about registration as an inpatient at Englewood Community Hospital.
Please bring a list of your medications with you to the Hospital, including product, dosage and frequency. This is important information for our staff to have in providing services requested by your physician. It is not necessary to bring the actual medications, but rather, just the list of information described above.
During registration / admission, an identification band will be placed around your wrist. It is critical that the band be left in place, until you are discharged from the Hospital. This allows our staff to make positive identification, so we can deliver the plan of care as prescribed by your physician.
Because we cannot be responsible for lost personal possessions, please minimize the number of personal valuables you bring to the Hospital. Especially with items such as finer jewelry, we strongly recommend you leave them at home, or send them home with a visitor.
The Outpatient Services Department at Englewood Community Hospital is located in back of the facility. For scheduling and information, they may be reached by phone at 941-473-5022. The registration desk within the Department allows patients to access the many key services affiliated with that area, namely:
Day Surgery
Radiology
Cardiac Catheterization
Endoscopy
IV Therapy
All other outpatient services may be accessed directly through those respective departments, including:
Laboratory
Open M.R.I.
Rehabilitation Services: Physical, Cardiopulmonary, Occupational, and Speech
Wound Care Center
